distance-sensitive and therefore each service location must be prequalified
to ensure service availability
as well as service speeds for that location. For this purpose both
the physical address (which may not be a P.O. Box) and telephone
number at the DSL service location are needed. A specific street address
is required to approximate distance from the DSL service location
to the Telco Central Office. A phone number is needed for the service
location to determine if the Telco Central Office has DSL facilities
